Notes<\/a>\n\t\t\t\t\t<\/div>\n\n\t\t\t\t\t<div id=\"elementor-tab-content-2283\" class=\"elementor-tab-content elementor-clearfix\" data-tab=\"3\" role=\"region\" aria-labelledby=\"elementor-tab-title-2283\"><p><strong>What if a person cannot get a Wellness Exam?<\/strong><\/p><p>There are a few types of people who will not be able to get a wellness exam (pregnant, people with ongoing serious health conditions).<\/p><p>Pregnancy \u2013 please evaluate whether or not it is reasonable for the person to get an exam in time. For example, if the due date is prior to 4\/1, the person should be able to reasonably get an exam before 6\/30.<\/p><p>Serious health condition \u2013\u00a0a person who has a serious health condition may be able to get a regular wellness exam to check other items. For example, a person with breast cancer could still get a wellness exam to check blood glucose, cholesterol, or have other appropriate preventive screenings.<\/p><p>Procedure to *waive wellness exam &#8211; Send an e-mail to SBA with the following information:<\/p><ul><li>Employee\u2019s name and UIN<\/li><li>Expecting mother\u2019s name (covered spouse)<\/li><li>Name of provider (OB\/GYN)<\/li><li>Date of first visit with diagnosis of pregnancy<\/li><li>Expected delivery date<\/li><\/ul><p>*The wellness exam will count as one step of the two-step wellness program.
